Community Navigators - Family & Seniors

The Community Navigator – Family & Seniors position collaborates with city departments and external organizations to coordinate information and referral services, outreach, and programming to reach individuals and families in need of services and support within Commerce City. To provide individualized services/meetings, especially to the most vulnerable residents. 

Purpose

The Community Well-Being team strives to be a centralized resource for individuals, families, and seniors experiencing housing insecurities or who are at risk for homelessness. Upon referrals or direct requests, the Community Navigator for Family & Seniors is responsible for contacting the individual or family at risk for homelessness, learning about their situation, and finding resources that can help individuals/families avoid displacement.

Goals

  • Build relationships and proactively work with individuals and families experiencing housing insecurities to avoid homelessness
  • Act as an advocate for city residents to ensure safety, stability and community
  • Partner with agencies, non-profits, and community organizations to meet the community's needs related to food, shelter, clothing
  • Create and build a program unique to Commerce City to address low income needs and poverty
